About Your Grace LLC
Your Grace LLC was founded on the belief that healthcare, transportation, and safe living environments are fundamental human rights. We are committed to creating opportunities and solutions grounded in respect and self-determination for individuals and our community. For years, our founder has opened their home to people with addictions, behavioral disorders, homelessness, elders, and peers, recognizing that grace is something our whole country needs right now.
We provide comprehensive, all-inclusive services designed to support individuals facing housing instability, chronic conditions, mobility challenges, and special needs. Our integrated approach combines non-emergency medical transportation, supportive housing, case management, meals, medical arrangements, home health assistance, and cognitive and behavioral therapy. Each of our residential facilities features single and double rooms, accessible amenities, 24-hour supervision, and wheelchair-accessible transportation to ensure dignity and comfort for all residents.
Our Services
Non-Emergency Medical Transportation
Wheelchair-accessible transportation services for individuals requiring assistance getting to and from medical appointments. We handle every aspect of transport from initial pickup to final destination delivery, serving those with chronic conditions and mobility challenges.
Supportive Housing Program
Non-time limited affordable housing with comprehensive support services including case management, meals, home health assistance, and 24-hour supervision. Designed for individuals facing homelessness, veterans, those with mental health issues, disabilities, and substance abuse challenges.
Integrated Support Services
All-inclusive residential services including cognitive and behavioral therapy, medical arrangements, lite housekeeping, and access to day rooms and communal facilities. Our supportive living environment provides the stability and care needed for individuals to achieve housing stability and improve their overall well-being.
